This is horrible and Owl thats what I do I read books and I know things shirt. We are going to lose great nurses and doctors because they will be afraid of making a mistake and either the stress will kill them or they will just say, no more I’m done. I’m using this in my Quality Improvement Class to prepare a cause-and-effect diagram. So sad that this Nurse is facing so Owl thats what I do I read books and I know things shirt. I was in Quality for many years. It appears there were several system failures that occurred. The first rule of investigation is to determine the root cause by looking at all possibilities. Even though there’s usually a human component fishbone diagram the root cause is almost always something in the system. Blame the process, not the person.
